A requisition from a licensed practitioner is required for your scan, but you do not need it to send this form. If you cannot upload it now, bring it with you to your appointment.

MRI safety questionnaire

These questions keep you safe in the scanner, and a technologist reviews every answer. Please answer what you can — if you are not sure about something, say so, and skip anything you cannot answer. We would rather call you than have you guess.

Do you have a cardiac pacemaker or implanted defibrillator (ICD)?

Do you have any other implants or devices? (cochlear implant, neurostimulator, medication pump, aneurysm clip, heart valve, stent, joint replacement, etc.)

Have you ever had metal fragments in your eyes or body? (welding, grinding, shrapnel)

Have you had surgery in the last 6 weeks?

Are you pregnant, or is there any possibility you may be pregnant?

Do you have kidney disease, or are you on dialysis?

Have you ever had a reaction to MRI contrast (dye)?

Do you experience severe claustrophobia?

Is your weight over 440 lb (200 kg)?

Is there anything else the technologist should know before your scan?
